The Students' Union Health and Dental Plan is mandatory for full-time students. However, if you already have coverage through a partner, a parent, or your workplace, you can opt-out of the plan.
You can opt-out online, pick up an opt-out form from the Student's Union office, room 150 in the cafe, or download the attached form. Completed forms must be returned to the Students' Union office before 4:00 pm, September 30, 2010. Scanned forms can be emailed to su@ecuad.ca. Forms can be faxed to 604.844.3801.
Proof of alternative coverage is required, and consists of your insurance company name, and group or plan number. Please ensure that you DO NOT give out your certificate ID, Social Insurance number, or any other identifying number as this information is private and confidential.
